How Do Oral Appliances Work?

Oral appliance therapy is a well-established approach for addressing obstructive sleep apnea (OSA) and snoring, and for the right patient, it can make a significant difference in sleep quality, energy, and overall health. The Problem Oral Appliances Are Designed to Solve


During sleep, the muscles throughout the body relax, including those in and around the throat. For many people, this relaxation allows the tongue and soft tissues at the back of the throat to shift backward, narrowing or blocking the airway. When the airway narrows but stays open, the movement of air through the constricted space causes the vibration known as snoring. When the airway collapses completely, breathing stops until the brain triggers a partial arousal to restore airflow. This is obstructive sleep apnea.

These interruptions affect sleep architecture, meaning they prevent the body from spending adequate time in the restorative stages of sleep. Over time, untreated sleep apnea is linked to elevated blood pressure, cardiovascular stress, daytime fatigue, and difficulty concentrating.

The Core Mechanism: Moving the Lower Jaw Forward


Oral appliances work by repositioning the lower jaw during sleep. The most widely used design is the mandibular advancement device, which consists of two custom-fit trays, one for the upper teeth and one for the lower, connected in a way that holds the mandible in a slightly protruded position.

This small change in jaw position has a direct effect on the airway. As the lower jaw moves forward, the base of the tongue and surrounding soft tissues are pulled away from the posterior pharyngeal wall. The result is a larger, more stable airway that is less likely to collapse during sleep. Snoring is reduced or eliminated because the airway is no longer narrow enough to produce significant vibration, and for patients with obstructive sleep apnea, the apnea events decrease or stop entirely.

The level of jaw advancement is customized for each patient. Most devices are adjustable, allowing the treating provider to incrementally increase or decrease advancement until the optimal therapeutic position is reached.

The Role of a Dental Provider in Oral Appliance Therapy


While oral appliances are worn in the mouth, they address a medical condition. The process typically involves collaboration between a sleep physician, who diagnoses and monitors the sleep disorder, and a dentist trained in dental sleep medicine, who fabricates and manages the appliance.

Before a device is made, the dentist evaluates:
•  The health and stability of the teeth and supporting structures
•  The range of jaw movement and presence of any temporomandibular joint concerns
•  The bite relationship and how it may be affected by sustained jaw advancement
•  Whether the patient's anatomy and sleep apnea severity are appropriate for this treatment approach

Impressions or digital scans are taken and sent to a dental lab where the appliance is fabricated to exact specifications. This precision matters because a poorly fitting device will not maintain its position through the night and will not produce consistent results.

Adjusting to the Appliance


Most patients find oral appliances more comfortable and easier to tolerate than continuous positive airway pressure (CPAP) machines, though there is still an adjustment period.

During the first few weeks, some patients experience:
•  Mild jaw or tooth soreness as the muscles adapt to the new position
•  Increased saliva production or, conversely, dry mouth
•  Temporary changes in how the upper and lower teeth meet in the morning

These effects are typically short-lived. Morning jaw exercises, often provided alongside the appliance, help the bite return to its normal position after the device is removed. If discomfort persists, the advancement can be reduced slightly and re-titrated more gradually.

Confirming the Appliance Is Working


Feeling better is a positive sign, but it is not a substitute for objective confirmation. After the device has been titrated to the target position, a follow-up sleep study, either conducted in a lab or with a home sleep testing device, confirms whether apnea events are being adequately controlled. If the initial results show partial improvement, further adjustments are made and the study is repeated.

This follow-up process is an important part of responsible oral appliance therapy. Snoring may improve even when apnea events continue, so subjective feedback alone is not sufficient to confirm treatment success.

Long-Term Care and Maintenance


Oral appliances require regular cleaning to prevent bacterial buildup. Most providers recommend rinsing the device with cool water after each use and using a soft toothbrush or device-specific cleaning tablets periodically. The appliance should be stored in a protective case when not in use.

At routine dental visits, the fit and condition of the appliance will be checked. Over time, minor changes in tooth position or jaw structure can affect how the device fits, and periodic adjustments or remakes may be necessary. With proper care, most appliances remain functional for several years before replacement is needed.
